Appointments:

This clinic runs an appointment system. Please telephone the surgery in advance to make your appointment. If your matter is urgent, please advise the receptionist and you will be dealt with promptly (with advice from our nurse if necessary). Long consultations are available if required ( please advise receptionist at time of booking ). You are encouraged to request an appointment with your 'regular' Doctor to ensure continuity of care. In the event you are unable to attend an appointment or change an appointment, it is requested that you give 24 hours notice. Please note if you fail to attend an appointment fees will apply ($70.00 for standard consult missed, $120.00 for long consult- eg. new patients, care plans- missed). This fee is payable prior to any further bookings being scheduled

Home and Nursing home visits:

These are available for our regular patients who are too unwell to attend the surgery (conditions apply).

Recalls & Reminders:

This practice routinely sends recall and reminder letters/sms to patients regarding further consultations and assessments. This is particularly useful for such things as pap smears and immunizations. If investigations are ordered by your doctor, it is your responsibility to ensure that you obtain these results. It is recommended that you make a follow-up appointment to discuss those results. In the case of abnormal results we will make all effort to contact you Therefore it is essential that you keep your contact details with us current.

AT EACH VISIT OUR RECEPTIONIST WILL ASK YOU THE FOLLOWING QUESTIONS FOR IDENTIFICATION and to ensure the above details are current.

Telephone and email contact with Doctors:

It is difficult for the doctor to interrupt a consultation with a patient to accept a phone call. You will generally be asked to leave a message and the doctor will return your call when he becomes available. If the matter is urgent please advise the receptionist and your call will be directed to Doctor or nurse if applicable. If you wish to contact your doctor by email please ask reception who will contact the doctor for permission to disclose the doctors email.

Repeat presciptions:

At the doctors discretion, repeat prescriptions may be provided without a consultation (conditions & fee applicable). Please ask reception for details.

Centrelink forms, referrals, insurance forms etc

require a consultation with the doctor for completion. (fees may apply) Please ask reception for details.

Lost forms:

Please keep your Xray, pathology and referral forms/letters safe and secure. If you misplace or forget to take your forms to your appointments a fee may be charged for re-issue.
