A SECURE ONLINE SUMMARY
A personally controlled My Health Record is a secure online summary of your health information. You control what goes into it, and who is allowed to have access. It gives you more control over your health information than ever before, placing you at the centre of Australia’s health system.
HOSPITALS, DOCTORS AND HEALTH CARE PROVIDERS
Your My Health Record allows you and your doctors, hospitals and other healthcare providers to view and share your health information to provide you with the best possible care. Aspire Medical centre encourages the use of My Health Record across our catchment.

MY AGED CARE

The Australian Government is implementing reforms to the aged care sector, including the referral process, and how patients’ access aged care services. General practitioners, allied health professionals, consumers, carers, family and friends should now contact My Aged Care on 1800 200 422 as their first point of entry and referral. My Aged Care will then assist the person to access the right service at the right time. This could be a Commonwealth Home Support Program or a visit from the Aged Care Assessment Team.

ADVANCED CARE PLANNING

Advance care planning offers people the opportunity to discuss with family members, carers, friends and health professionals the choices for care at the end of life. The advance care plan Statement of Choices form focuses on your wishes and choices for health care into the future. Your advance care plan, including your Statement of Choices form, will only be used to help guide health care decisions if you are unable to make or communicate your decisions.
You can change your mind, your plans and legal documents at any time. You can complete your advance care plan for yourself. However, it is a good idea to complete this together with your doctor. You can also complete an advance care plan for someone else if they do not have the capacity and if you are their recognised substitute decision maker.
